Register For Athletics

We provide the convenience of online registration for all of our athletic teams through FINALFORMS (https://enfield-ct.finalforms.com). FINALFORMS is a secure registration platform that provides you with an easy, user-friendly way to register for our programs, and helps us to be more administratively efficient and environmentally responsible. When you register through FINALFORMS, the system keeps track of your information in your FINALFORMS profile.
You enter your information only once for each family member for multiple uses and multiple programs.

STEP 2:  CONFIRM YOUR PHYSICAL DATE

Physical Exam Requirements for Sports Participation in Grades 6-12  

To participate in any sport at any level, all students are required to have a physical exam, conducted by their health care provider (physician, physician’s assistant, APRN or RN), on record in the Nurse’s Office.

This physical exam is good for 13 months.

IMPORTANT NOTE:
This physical must be on record PRIOR to try-outs: there are no exceptions.

If a student’s exam expires in the middle of the particular season, he or she may begin the season but must have an updated physical by the expiration date.

Points to Remember:

• Due to the large number of students who participate in sports, it is often difficult to get an appointment with your health care provider on a last minute basis.  Parents whose children plan to participate in a sport are urged to make an appointment with their health care provider well in advance of the intended sports season.
• Student-athletes must be registered in FinalForms for physical information to be added to their file.
• If you submitted your physical to the nurse prior to registering for a sport, the nurse will need to be notified of this in order to update your file.
• Processing of the physicals may take up to 1 week but our nurses will do everything in their power to process them quicker.
• Physicals that are turned in the day of or day before tryouts may not be processed in time for the athlete to participate.

Sports Examination Form

The Sports Evaluation Physical Form can be picked up Enfield High School in the main office.   You can also find a copy of the physical form on the following website. (you will only need pages 1 & 3)   http://www.casciac.org/pdfs/physical.pdf

Physical forms must be turned in and processed by the School Nurse.  Be sure to either drop off or fax in physicals.  Emailed copies will not be accepted. 

SUMMER NURSE PHYSICAL UPDATES

The EHS nurses will be available on August 1oth and 14th from 7:30 am – 3:30 pm to update physicals for those who are registered for a fall sport.

Coaches will not accept and cannot approve the validity of this information.

(Middle School physicals can be mailed or dropped off once school begins.)
